Action Plans
Some students have medical conditions (food/insect allergies, asthma, diabetes, seizures/epilepsy) that require specific medical care while at school. Because these medical conditions can lead to severe reactions and or medical events at school, school nurses often request additional information from the student’s health care provider regarding their condition to help the nurse better care for the student at school.
Should a student require medical care at school for food/insect allergies, asthma, diabetes and/or seizures/epilepsy, an action plan and Authorization for School Medication Administration form must be completed by the student’s health care provider and parent/guardian and submitted to the School Nurse.

To ensure diabetes management needs of students are met, parents/guardians are encouraged to work with the student’s health care provider to obtain a diabetic action/emergency plan of care and medication orders.
